FMLA and other medical leave certification may be completed for established patients when the clinical record supports it. Approval of leave is decided by the employer or plan administrator.

IEP supporting documentation and Section 504 supporting documentation/forms, including ADHD-related classroom and testing accommodations, may be completed when clinically supported. Schools and testing organizations make the final decision. See Forms and documentation.

For minors, parent or guardian involvement and consent are handled according to age, clinical circumstances, and applicable state law. Adolescents may also have appropriate private time with the clinician.

Not always. Medication is considered when it is clinically appropriate. A first visit does not guarantee a prescription or paperwork.
