Clinical Documentation & Support
Paperwork should serve the care, not replace it.
When paperwork matters, the clinical picture matters too.
Sometimes mental health symptoms affect more than how someone feels. They may affect work, school, attendance, concentration, daily functioning, or the ability to meet certain responsibilities. In those situations, patients may need clinical documentation from a treating healthcare professional.
Avenelle can consider documentation requests when they are clinically appropriate and supported by the established treatment relationship and available clinical information.
What documentation may be considered?
Depending on the circumstances, applicable requirements, and clinical findings, documentation requests may relate to workplace accommodations, educational accommodations, FMLA or medical leave documentation, return-to-work considerations, school-related forms, and other clinically appropriate requests.
Not every form or request falls within the scope of psychiatric practice, and additional evaluation or records may sometimes be necessary.
Why can’t paperwork simply be completed on request?
Because signing a form is a clinical statement. When a clinician documents that a psychiatric condition causes certain functional limitations, that statement should be supported by the evaluation, treatment history, clinical findings, and information reasonably available to the clinician.
For that reason, Avenelle does not guarantee completion of documentation simply because a form has been requested.
Documentation does not determine the final decision.
Employers, schools, testing organizations, government programs, insurers, and other entities establish their own eligibility criteria and make their own decisions. Clinical documentation from Avenelle does not guarantee approval of FMLA, medical leave, an IEP, a 504 Plan, workplace or academic accommodations, disability benefits, or another requested outcome. Additional fees, appointments, records, or processing time may apply depending on the complexity of the request and applicable practice policy.
Ready to take the next step?
We can first determine whether the request falls within the scope of your psychiatric care and whether sufficient clinical information is available to support it.
