Forms and documentation
For established patients, Avenelle may provide medical certification when supported by the clinical record. Completing a certification does not guarantee approval of leave. Eligibility and leave determinations are made by the employer or plan administrator under applicable federal, state and employer requirements.
When an evaluation supports the need, Avenelle may complete clinical documentation for school and testing teams. The school district develops and implements an IEP. Avenelle does not create or run a student’s IEP or 504 plan.
